Here is the official IR FORM that MUST BE used to use an IR spot. PLEASE FILL OUT COMPLETELY. We will not accept incomplete forms.
You will need to submit an IR form no less than 24 hrs in advance for that IR replacement to be able to play. Once a guy goes on IR they will not be able to play for the remainder of that season.
